چکیده

Background: Subtrochanteric fractures are one of the common fractures encountered in today's orthopaedic practice. Choice of implant still remains with the surgeon. The advantages and disadvantages of the Gamma nail have been well established in several studies done in the past, usually by comparing the results with the Dynamic hip screw (DHS). This study deals with the comparison of Proximal femoral nailing (PFN) versus Dynamic hip screw (DHS) in treatment of subtrochanteric fractures. Methods: This study was conducted in M. S. Ramaiah Hospitals from October 2007 to August 2009. During this period 50 cases of adult patients with subtrochanteric fractures of femur were selected according to the inclusion criteria and classified according to Seinsheimer's classification. Results: We found that PFN was superior to DHS in many ways such as reduced intra-operative blood loss, lesser operative time, reduced radiation exposure, less amount of limb shortening, reduced hospital stay, lesser infection rates and early mobilization. Conclusion: Nailing has the advantage of providing rotational as well as axial stability in cases of subtrochantric fractures allowing a faster postoperative restoration of walking ability, when compared with the dynamic hip screw.

Type II Intertrochanteric Fractures: Proximal Femoral Nailing (PFN) Versus Dynamic Hip Screw(DHS)

Background: Intertrochanteric fracture is one of the most common fractures of the hip especially in the elderly with osteoporotic bones, usually due to low-energy trauma like simple falls. Dynamic Hip Screw (DHS) is still considered the gold standard for treating intertrochanteric fractures by many. Not many studies compare the DHS with Proximal femoral nail (PFN), in Type II intertrochanteri...

Pertrochanteric femoral fractures treated with a dynamic hip screw or a proximal femoral nail. A randomised study comparing post-operative rehabilitation.

We treated 108 patients with a pertrochanteric femoral fracture using either the dynamic hip screw or the proximal femoral nail in this prospective, randomised series. We compared walking ability before fracture, intra-operative variables and return to their residence. Patients treated with the proximal femoral nail (n = 42) had regained their pre-operative walking ability significantly (p = 0....

Management of Pertrochanteric Fractures with Proximal Femoral Nail and Comparison of Results with Dynamic Hip Screw

Introduction: Proximal Femoral Nail and Dynamic Hip Screw, both are the accepted implants for the internal fixation of Pertrochanteric fractures. Objectives of the research were to compare the results of proximal femoral nail and dynamic hip screw in terms of functional mobility of the patients and the results would also be compared in terms of operating time, peri-operative blood loss, radiolo...

Finite Element Analysis for the Treatment of Proximal Femoral Fracture

Dynamic hip screw and gamma nail have been widely used to treat the patients with proximal femoral fractures, but clinical failures of those implants are still to be found. This study developed three-dimensional finite element models to investigate the biomechanical performances of the implants. Two kinds of commercially available implants (dynamic hip screw and gamma nail) and one newly design...

Comparing Therapeutic Outcomes of Dynamic Hip Screw and Multiple Cannulated Screws as Internal Fixation in Femoral Neck Fractures

Background and purpose: Femoral neck fracture is associated with a high rate of side effects, especially in young people, who are usually affected by severe energy shocks. Results after injury depend on some factors, including displacement, fragility, bleeding rate, proper placement, and stable and strong fixation. The aim of this study was to compare dynamic hip screw (DHS) or multiple cannula...
